How Do Same-Day Crowns Work?

Same-day crowns use in-office CAD/CAM (computer-aided design and computer-aided manufacturing) technology to design and mill your restoration in a single appointment. Rather than taking a physical impression and sending it to a dental lab, we use a digital intraoral scanner to capture a precise 3D image of your tooth. That digital model is then used to design your crown on-screen, and an in-office milling machine carves it from a ceramic block while you wait.

The entire process, from tooth preparation to final cementation, typically takes around two hours. You leave the appointment with a permanently placed crown and no temporary restoration to manage in the meantime. For patients with busy schedules or those who simply prefer to get procedures done in as few visits as possible, the convenience factor is significant.

What Are the Pros of Same-Day Crowns?

The advantages of same-day crowns go beyond just saving a trip to the office. Here are the key benefits worth knowing:

  • No temporary crown: Traditional crowns require a temporary restoration between appointments, which can crack, fall off, or cause sensitivity. Same-day crowns eliminate this entirely.

  • All-ceramic material: Same-day crowns are milled from ceramic, which blends naturally with surrounding teeth and contains no metal.

A 2023 systematic review and meta-analysis published by the National Institutes of Health found that CAD/CAM-fabricated crowns demonstrated favorable clinical outcomes comparable to conventionally manufactured restorations across multiple performance measures. Are There Any Cons?

Same-day crowns work exceptionally well for the majority of cases, but there are some clinical situations where a traditional crown may be better suited. Because in-office milling uses ceramic exclusively, same-day crowns are not ideal for patients who need metal or porcelain-fused-to-metal restorations for specific strength requirements. Additionally, digital scanners can struggle to capture the full picture when a fracture line or crown margin extends significantly below the gumline. In those cases, a traditional lab-fabricated crown allows for more customization and material flexibility. If the patient is short on time, a traditional two-step crown may be better. 

The good news is that the same CAD/CAM machine can mill a perfect temporary in 10 minutes, allowing our CAD/CAM in-office mill to still provide a better two-step crown result than traditional methods. 

How Do Traditional Crowns Work?

Traditional crowns follow a multi-appointment process. At the first visit, the tooth is prepared and a physical or digital impression is taken, a temporary crown is placed, and the impression is sent to an off-site dental laboratory. The lab technician fabricates your permanent crown over approximately one to two weeks, and you return for a second appointment to have it permanently cemented. This process has been the standard in general dentistry for decades and remains effective for a wide range of clinical needs.

Traditional crowns offer greater material versatility. They can be made from metal alloys, gold, porcelain-fused-to-metal, or full porcelain depending on the location of the tooth and the patient’s specific oral health picture. For teeth that endure heavier bite forces, or for cases involving severe structural damage, the lab environment allows a skilled dental technician to construct a restoration with a high degree of detail and customization.

The Main Drawback

The primary challenge with traditional crowns is the time commitment and the temporary crown period in between. Temporary crowns are made from a weaker acrylic material and are designed to last only a few weeks, during which they can occasionally loosen, crack, or cause sensitivity to temperature. Patients must also be careful about what they eat and how they care for the temporary restoration until the permanent crown arrives. For patients in a mountain community with demanding schedules, squeezing in two appointments weeks apart can be genuinely inconvenient.
